Sovereign Hospice brings ALL services to your “home” whether that be a residential home, group home, assisted living, nursing home, hospital or inpatient hospice unit. We work with all major nursing homes, hospitals and physician groups. We can adapt to the changing needs of the health care industry with the ultimate goal of taking care of the dying patient and family.

In most cases, all services are paid for by insurance but we do have specialized programs that extend our hospice program to uninsured patients as well.

As seasoned professionals in hospice and palliative care, we understand that death and dying brings a group of expected symptoms such as pain, anxiety, restlessness, shortness of breath, depression, constipation, lack of appetite, increased sleep, fatigue and eventual unresponsiveness. Hospice professionals are trained to anticipate, mitigate and teach family on upcoming signs and symptoms of impending death. We areĀ  comprised of physicians, nurses, social workers, hospice aides, chaplains, bereavement coordinators, volunteers, pharmacists, medical supply and durable equipment technicians. This is the highest level of care any person can receive at home and our licensed professionals can do everything that a hospital can do for you as it relates to end of life care, in your home.

Sovereign Hospice considers itself to be a guardian of our Medicare Trust Fund. We aim to prevent un-necessary hospitalizations and treatments especially on an older, frail patient with multiple co-morbids facing terminal conditions. In case of any emergency, our on call registered nurses can come to your home within an hour of you needing us. We offer 24 hour on call services 7 days a week, 365 days a year and open on national holidays. Our pharmacy and DME company is open 24 hours and can deliver any equipment or medication that is needed for patient comfort within a matter of hours. All hospice services are offered according to the scope and frequency ordered by the Interdisciplinary Team which meets once every 15 days and comprises of all disciplines involved in the care of the patient – including the patient and family and the attending physician chosen by the family. We provide comprehensive and holistic care covering all of the patient and family needs.

A highly specialized hospice team consisting of MD, RN, LMSW and Chaplain is on call 24-hours a day, 7 days a week to meet any of our after hour patient needs. We have a zero percent re-hospitalization rate for the last two consecutive years. We utilize the latest in telemedicine, triage and tracking for our high risk patient groups and front load nursing visits to prevent a return to acute care.

Sovereign Hospice serves all counties within the Dallas-Fort Worth Metroplex. Our response time for an on call RN visit is currently 30 – 45 minutes.
